What does it include in the template?

The template is already preconfigured. It includes:

  1. Landing with main and background image, which you can modify, and a field to add a description to explain the action and how to participate. Data form with the fields (name, surname, and email) the participant must fill in to participate.
  2. Participation form: Question to measure customer satisfaction on a scale from 'Very dissatisfied' to 'Very satisfied'. Multiple choice questions, so the user can indicate the reasons for their previous rating.  Question with open response option, so the user can provide more information.
  3. Thank you page.
